251-bed acute care hospital
Provider will also work at a secondary clinic nearby
Provider will work with another GI physician in the practice

Credentialing / Clinical Activity Requirements
Must provide documentation of gastroenterology services representative of the scope and complexity of privileges requested during the previous 24 months
If training to perform double balloon enteroscopy or capsule endoscopy occurred in the previous 2 years, applicant must provide:
Description of supervised training on human subjects
Letter from supervising physician attesting to clinical competence as an independent operator
If training occurred more than 2 years ago, applicant must provide documentation of ongoing clinical practice in each privilege requested
Requirements
Board Certified in Gastroenterology – required
Washington license or IMLC – required
BLS – required
ACLS – required
DEA – required
Epic experience – required
Minimum 2 years of experience – required

ERCP is not required, but if a provider performs ERCP, it should be clearly noted in the presentation subject line and presentation form.
